Benefit Summary & Analysis Report**($471)
This is comprehensive report provides information on how work and earnings may affect benefits. Make sure that your return to work is an INFORMED choice. This report will address all state and federal programs.
In-Service Training on Social Security Retirement, Disability, & Work Incentives
My team and I will work with you and your organization to tailor a training specifically addressing the needs and concerns of your staff and client base. A sample of topics we provide training on include:

PASS Plan Preparation**($250/$50 an hour for additional assistance)
The Plan for Achieving Self-Support is an excellent work incentive that allows you to use your income or other things you own to help your reach your work goals. An evaluation of eligibility for the PASS program will be provided free of charge. If it determined that the PASS Plan is a viable option, this service will include the completion of the SSA-545, Application for the PASS Plan, and guidance on how to make sure the plan is a success. Note: This does NOT include the creation of a business plan. A completed business plan is required for all PASS Plans that include a self-employment goal. Personal Benefit Services ($150)
This is a one-on-one service to provide guidance to individuals and their families on what they need to know to about Social Security and SSI benefits and how benefits apply to them. This is not as comprehensive as a Benefit Summary & Analysis Report, but instead is a roadmap to navigate families through the SSA/SSI maze. The report may include (but is not limited to):
- A review of current eligibility for SSI/SSDI benefits
- A plan for when it would be appropriate to file for SSI/SSDI/CDB
- Tips on best practices for filing for benefits based on disability
- Tips for eligibility to CDB benefits
- A review of Medicare eligibility
- A review of Medicaid eligibility based on SSI benefits
- A review of work incentives appropriate to individual
